Autism Levels Explained: What Levels 1, 2, and 3 Really Mean, and Why the Root Cause Goes Deeper Than Genetics
Episode 142, Experience Miracles Podcast | Host: Dr. Tony Ebel, DC, CACCP, Pediatric Chiropractor & Founder of PX Docs | Published: September 26, 2024 | Duration: ~22 min
Key Takeaways
- Autism affects 1 in 36 children today, and the number keeps rising, yet the most common medical explanation (“it’s genetic, we’re just diagnosing it better”) is, according to Dr. Tony Ebel, scientifically inaccurate and fails to address what is actually causing children to struggle.
- The DSM’s three autism levels describe surface severity, support needs, communication, behavior, but do not identify root causes, leaving level 2 and level 3 families without answers or a path forward.
- Dr. Ebel’s Perfect Storm framework identifies three sequential triggers, prenatal distress, birth trauma to the brainstem, and toxic overload, as the actual drivers of autism, particularly for more severe cases.
- Level 2 and level 3 autism are differentiated from level 1 not just by symptom severity, but by how deeply nervous system dysfunction, dysautonomia, and subluxation have become embedded in the autonomic and central nervous system.
- Neurologically-Focused Chiropractic Care aims to release stuck sympathetic fight-or-flight stress and activate the vagus nerve and parasympathetic system, the side of the nervous system responsible for regulation, communication, social-emotional connection, and anti-inflammatory function.
What Do the Three Levels of Autism Actually Mean for Your Child’s Health?
Autism spectrum disorder is not a single, uniform condition, it spans three levels defined by the DSM (Diagnostic and Statistical Manual), each reflecting a different degree of severity and support need. But while those levels help families access services like early intervention, IEPs, and insurance coverage, they do not explain why a child ends up at level 2 or level 3 in the first place.
Dr. Tony Ebel argues that the real differentiator between mild and severe autism is not primarily genetic or random, it is the depth of nervous system dysfunction a child is carrying. Level 2 and level 3 children have had their autonomic nervous system driven into a more severe, more deeply locked state of sympathetic dominance, essentially, a nervous system permanently stuck in fight-or-flight. The vagus nerve and parasympathetic system, which govern rest, digestion, immune regulation, speech, and social-emotional connection, are suppressed or shut down entirely.
For parents navigating level 2 or level 3 autism, this reframe matters: the question is not just “how do we manage symptoms?” but “what created this level of nervous system dysfunction, and can it be addressed at the root?” Dr. Ebel’s answer is yes, and INSiGHT scanning technology gives practitioners a way to measure that dysfunction directly, independent of a child’s diagnostic label.
Autism Prevalence and Why the Neurodiversity Framing Falls Short for Severe Cases [00:00:00 – 00:02:30]
Dr. Tony Ebel, DC, CACCP: Autism is no longer rare. Today, one in 36 children is diagnosed, and that number keeps rising. In fact, those studies are always outdated because they’re looking backwards while the Perfect Storm of autism continues to move forward way too fast. And what’s even more alarming is how misunderstood this diagnosis has become online.
You’re going to hear a lot about neurodiversity and being proudly autistic. We believe in respecting and supporting every single child, but for families navigating the daily realities of level 2 autism and especially severe level 3 autism, the truth is this isn’t just a different way of thinking or a unique personality trait that these kids are struggling with.
These children truly struggle with their health in a multitude of ways, and their families are in a constant daily crisis. We’re talking about children who can’t speak, who can’t sleep, who self-injure and scream in sensory and digestive pain for hours, and they live trapped inside of a nervous system and inside of a brain and a body that is stuck in constant total stress overload.
“These children truly struggle with their health in a multitude of ways, and their families are in a constant daily crisis. We’re talking about children who can’t speak, who can’t sleep, who self-injure and scream in sensory and digestive pain for hours.”
And then parents and siblings who are running on empty trying to hold it all together and love and support their child and get them to heal the best they can. And yet the medical world, the social media world, all say, “Don’t worry. The root cause is genetic. There’s nothing we can do.” And then the functional medicine world awesomely steps up and says, “There’s more going on. Let’s look into the gut. Let’s look into the immune system.”
But what if there’s something even deeper, a foundational neurological issue that actually explains why so many of these level 2 and level 3 autism kids cannot connect, can’t regulate, can’t sleep, and can’t develop and live life the way that they are designed to?
Luke’s Story: How Neurologically-Focused Chiropractic Care Changed Everything [00:03:00 – 00:06:30]
Let’s start with a story about one of our best friends, Jackie, her husband Lee, and their little dude Luke.
Jackie is an incredible Perfect Storm autism mom who was always asking herself if her son would ever heal or be the same again. Her beautiful son Luke started regressing at about age 13 months. He had completely stopped talking. He stopped making eye contact, was often inconsolable, and had a hard time sleeping. Tons of sensory challenges and, honestly, he just wasn’t connecting well with others at all. It was an extremely difficult time for Luke and his entire family.
But being the powerhouse, all-in family that they are, they went all in on getting Luke the help he needed to get out of that regression and into progression once again.
What his mom said was that the biggest impact, through all the different therapies and treatments they tried, was getting their son adjusted and under Neurologically-Focused Chiropractic Care. Luke was in early intervention. He was getting the therapies. They were trying all the other things right alongside chiropractic, but it truly has been chiropractic that is the number one game changer that has helped Luke go from struggling to thriving. He now can calm, he can retrain his nervous system, he can regulate his emotions, and his brain and his body are connected so that he could begin to heal on a deeper, more foundational, more complete level.
She said, “I knew it was working when we started to be able to get back to some of the parts of our life that were taken away when he slipped away from us into autism regression”, eye contact, being able to calm down, sleep and regulate. But most importantly, when he started talking again. And if you’ve ever had conversations with Mr. Luke, they are the best conversations ever.
“Once we figure out the exact root causes and we get to the foundational layer of nervous system dysfunction, not just blaming genetics, not just looking at the gut only, but digging even deeper into the nervous system, you better believe that recovery is possible.”
Autism is a neuro-biomechanical, neuro-biomedical struggle for so many kids. It has a multifaceted root cause. And the more of these Perfect Storm triggers and struggles that a child goes through, the more severe their challenges become and the more likely they move into level 2, level 3 autism.
Breaking Down the Three Levels of Autism (DSM Criteria) [00:08:00 – 00:10:30]
First, let’s talk about the three levels of autism according to the DSM, the diagnostic manual the medical world uses.
When you have a chronic, multifaceted, multi-root cause, multi-systems disorder like autism, it goes beyond what a traditional pediatrician, and honestly even a traditional neurologist, can really handle. They have no training in digging deep into root cause. The entire medical system is about taking a survey, doing some exams, finding symptoms, putting them into a box, calling it a condition, and then giving it a medication and/or therapies and treatments. None of that gets to the root cause.
Level 1 autism: These children require some support for social communication. They might be inflexible with transitions, prefer certain clothes, certain foods, certain routines. Level 1 certainly comes with its challenges, and those unique challenges are real.
Level 2 autism: A child who requires substantial support. They have more pronounced sensory issues, communication challenges, neurodevelopmental delays, and repetitive behaviors, behavioral dysregulation and emotional dysregulation. Children with level 2 autism also struggle significantly with their neurobiological health. Their gut and digestive system is a mess. Their immune system is a mess. Their posture, movement, gait, tone, motor planning, all of it is dysregulated.
Level 3 autism: These are the severe cases that require very substantial, every-single-day support. They struggle severely with communication, many, most of them, are completely nonverbal. Very limited social interaction. Severe challenges with behavior and emotional dysregulation, digestion, neuroinflammation, motor tone and planning. Their whole system is stuck in sympathetic overdrive. And everything connected to the nervous system, which is everything in the body and everything in life, is severely challenged.
So while these levels help determine diagnostic insurance coverage, early intervention, school support, and community support systems, and those supports are absolutely essential, these levels do nothing to help parents figure out: why is my child still struggling? What is really causing all this?
Debunking the “Purely Genetic” Myth: What’s Actually Driving Autism [00:11:00 – 00:12:30]
The root cause that most traditional doctors miss is that it’s not just genetics. It’s not just bad luck.
If you want to talk about someone truly not understanding root cause and neurophysiological development, then meet a doctor who says that autism is purely genetic and we’re just diagnosing it better. That is the most outdated, unscientific, inaccurate description of what’s going on. Run to pxdocs.com and we will show you all the studies and all the reality.
“If you want to talk about someone really, truly not understanding root cause and neurophysiological development, then meet a doctor who says that autism is just purely genetic and we’re just diagnosing it better. Run as far and as fast as you can in the other direction.”
There is very little coming from genetics. There’s a lot coming from the gut. And there’s even more coming from stress dysfunction and something called subluxation, dysregulation, and dysautonomia of the central and autonomic nervous system.
People are told, “It’s the gut, it’s the microbiome, it’s leaky gut, it’s toxins.” By the way, all of that is accurate, and there’s also genetic predisposition. We don’t throw the baby out with the bath water on the genetic conversation. We just don’t blame everything on it. So: very little from genetics, a lot from the gut, and even more from nervous system dysfunction.
The Perfect Storm: The Three Triggers Behind Autism Severity [00:13:00 – 00:16:30]
The Perfect Storm is the catchall term and scientific theory Dr. Ebel first developed in 2008, first putting cases together in his practice, then finding the science to support it, then creating clinical protocols to help kids get out of the storm.
The Perfect Storm is this, first and foremost: there’s a series of early and often sequential triggers of stressors.
Trigger 1: Prenatal distress. Fertility challenges, high-stress pregnancies, high-intervention pregnancies, toxicity during pregnancy, fear and emotion and anxiety during pregnancy. In the literature, this is called maternal distress.
Trigger 2: Birth trauma. This is the biggest one that nobody is talking about, birth intervention, trauma, and injury to the brainstem via things like induction, forceps, vacuum extraction, and C-section.
Trigger 3: Toxic overload. Our kids are exposed to more toxins, inhaled, ingested, and injected, than ever before.
Those three progressive triggers set the storm in motion. Then the second phase unfolds: kids who are colicky, can’t latch, can’t nurse, can’t soothe, can’t sleep, can’t poop, refluxy, eczema, all these early challenges. Then they go into chronic ear infections, missed motor milestones, sitting, crawling, delayed walking, delayed talking, sensory issues, meltdowns, tantrums, inflexibility, continued sleep challenges.
The pediatrician says, “Don’t worry, they’ll grow out of those things.” No, they don’t. Those are all signs of a nervous system stuck in fight-or-flight, stuck in what’s called sympathetic dominance. They don’t grow out of colic and chronic ear infections and meltdowns and tantrums. They grow into neurodevelopmental challenges like autism.
“They don’t grow out of colic and chronic ear infections and meltdowns and tantrums. They grow into neurodevelopmental challenges like autism.”
And the more of those Perfect Storm stressors that the child had, and the more severe they were, the more likely they move into level 2, level 3 autism. Those early infant and childhood conditions, chronic ear infections, eczema, constipation, sleep difficulty, sensory challenges, meltdowns, neurodevelopmental delays, those are not something they grow out of.
Why Level 2 and Level 3 Autism Go Deeper, and How INSiGHT Scans Find It [00:17:00 – 00:20:00]
For a level 2 or level 3 child with autism, the nervous system dysfunction was driven deeper into the autonomic and central nervous system. It’s not something that will probably respond as easily to traditional early intervention like physical therapy, occupational therapy, speech therapy, or functional nutritional care like supplementation and diet changes.
What we find clinically, and what the research and scientific literature confirms, is that level 2 and level 3 autism have their root cause driven deeper into the foundation of nervous system dysfunction, dysregulation, and dysautonomia. We find more severe expression of subluxation. We find more imbalance, more inflammation, more sensory processing challenges.
That’s what makes INSiGHT scanning technology so powerful. We’re not looking to diagnose, neurologically focused pediatric chiropractors do not diagnose because we don’t treat autism. But the INSiGHT scans are, in our opinion, the number one test a family needs, more important than even the IEP level, the 504, all the essential things that get your family support. Because those things describe your child’s current function. The scans show you what’s really causing it.
The one thing you’ve been looking for this whole time, that deeper foundational layer, even below the GI map, even below the stool testing, even below the functional lab work, is nervous system dysfunction. And we have the testing to find it. Not to treat it. Not to cure it. But to get the stress off the body at that deep, severe dysfunction level, and help your child heal naturally, without drugs, on their own.
The Vagus Nerve, Parasympathetic Activation, and the Path Forward [00:20:00 – 00:22:00]
Your child is stuck in what’s called a sympathetic dominant fight-or-flight everyday storm. That’s the same as saying their vagus nerve and parasympathetic system, the side responsible for rest, regulation, restoration, speech and communication, social-emotional connection, anti-inflammatory function, and digestive motility, is injured, suppressed, and shut down.
Those Perfect Storm triggers activate and overstimulate the sympathetic fight-or-flight system, and they subluxate, interfere with, suppress, and shut down, the vagus nerve and the parasympathetic system.
Neurologically-Focused Chiropractic Care is not a treatment and it’s not a cure. It’s addressing the root cause. It’s making adjustments to release the stuck stress on the sympathetic nervous system and to activate and stimulate the vagus nerve and the parasympathetic system, the rest, repair, restore, regulate, anti-inflammatory, social, emotional, behavioral, speech and communication engagement side of the nervous system.
“Your child was designed to heal. They were designed to develop optimally. They’re designed to live life at its full expression, not with this level two, level three daily struggle. You don’t need another treatment and you don’t need a cure to get your child to the other side of the storm. You need to get to the deepest layer of the root cause.”
Level 1 kids have these same dysautonomia and dysregulation nervous system challenges, they just don’t have them as deep or as severe. So chiropractic care can improve quality of life across all levels.
A level 2, level 3 autism case has these nervous system dysfunctions deeper, more locked in, and more severe. So they need more chiropractic care, a higher frequency for a longer duration, to get out of the storm. But your child was not designed to be stuck. They are designed to heal. They are designed to live the highest quality of life possible. And that is entirely possible.
Frequently Asked Questions
What are the three levels of autism and what do they mean?
The DSM defines three autism levels based on the support a child needs. Level 1 children require some support for social communication and tend to be inflexible with transitions. Level 2 children require substantial support, with more pronounced sensory issues, communication challenges, and neurobiological dysfunction (gut, immune, motor). Level 3 is the most severe, children often completely nonverbal, with very limited social interaction, severe behavioral and emotional dysregulation, and whole-body sympathetic nervous system dysfunction. These levels help families access services but do not identify root causes.
What actually causes level 2 and level 3 autism?
According to Dr. Tony Ebel, the root cause is not primarily genetic. Level 2 and level 3 autism are driven by deep nervous system dysfunction, dysautonomia, and subluxation, particularly of the autonomic nervous system. The underlying mechanism involves the vagus nerve and parasympathetic system being suppressed or shut down, leaving the child locked in chronic sympathetic dominance (fight-or-flight). This is caused by a progression of Perfect Storm triggers: prenatal distress, birth trauma to the brainstem, and toxic overload.
What is the Perfect Storm and how does it cause autism?
The Perfect Storm is Dr. Ebel’s framework for understanding the sequential triggers that create autism, developed in 2008. The three triggers are: (1) maternal distress during pregnancy, stress, high interventions, toxicity; (2) birth trauma, brainstem injury from induction, forceps, vacuum extraction, or C-section; and (3) toxic overload, inhaled, ingested, and injected toxins. Children who experience more of these triggers, and at greater severity, are more likely to develop level 2 or level 3 autism.
Why don’t early infant symptoms like colic and ear infections get taken seriously?
Doctors often tell parents their children will “grow out of” colic, reflux, chronic ear infections, meltdowns, and sleep challenges. But Dr. Tony Ebel says these are not things to grow out of, they are early signs of a nervous system stuck in sympathetic dominance. Left unaddressed, children don’t grow out of them. They grow into autism, ADHD, and severe sensory challenges. These early warning signs are an opportunity to identify and address nervous system dysfunction before it becomes more deeply embedded.
What are INSiGHT scans and why does Dr. Tony recommend them?
INSiGHT scans are neurological assessment tools used by neurologically focused pediatric chiropractors to measure the function of the nervous system, specifically identifying the degree of sympathetic overload, dysautonomia, and subluxation. Dr. Ebel describes them as the most important diagnostic tool for families with level 2 or level 3 autism, because they reveal what’s really causing the dysfunction, something that genetic testing, lab work, and stool testing cannot show. The scans are not used to diagnose or treat autism, but to identify nervous system dysfunction so it can be directly addressed.
