ADHD in Children: Root Causes, Neuroscience, and Drug-Free Solutions

Episode 16, Experience Miracles Podcast | Host: Dr. Tony Ebel, DC, CACCP, Pediatric Chiropractor & Founder of PX Docs | Published: April 16, 2024 | Duration: 78 min

Key Takeaways

  • ADHD is not purely genetic, while family history creates a predisposition, no concrete genetic evidence exists for what researchers call “hardline genetic” causation. Environmental factors, prenatal stress, birth trauma, and early toxin exposure play a far larger and more direct role.
  • 3.3 million US children are currently on Class II stimulant medications for ADHD, drugs in the same regulatory category as cocaine, yet these medications suppress symptoms for only 4–6 hours and worsen the underlying neurological imbalance over time when discontinued.
  • The real root cause of ADHD is a central autonomic nervous system imbalance: too much sympathetic fight-or-flight activation (the gas pedal) and too little parasympathetic Vagus Nerve function (the brake pedal). This imbalance, not a chemical deficiency, is what throws dopamine and serotonin out of balance.
  • The Perfect Storm framework identifies four compounding factors that drive ADHD: high-stress pregnancy (maternal prenatal distress), birth trauma from interventions like C-section, forceps, or vacuum, early toxin and antibiotic exposure, and altered motor milestones, including skipping crawling, which the CDC quietly removed from its milestone chart.
  • Neurologically-Focused Chiropractic Care addresses ADHD at its actual neurological root by releasing tension from the sympathetic system and activating the vagus nerve, allowing the cerebellum, amygdala, hippocampus, and prefrontal cortex to function as designed, without medication.

What Actually Causes ADHD, and Why Medications Miss the Point

ADHD is not a broken brain, a genetic defect, or a chemical imbalance that requires lifelong medication. It is a neurological imbalance, specifically, a central and autonomic nervous system that has been pushed into a state of chronic Sympathetic Dominance, where the fight-or-flight system is stuck in overdrive while the regulating, focusing, calming side of the nervous system gets suppressed.

The chemicals that get out of balance with ADHD, dopamine and serotonin, are classified as neurotransmitters. Their entire function is to transmit messages from the nervous system. When the nervous system is imbalanced, these chemicals follow suit. Medications don’t fix the imbalance; they manipulate the chemical messengers while leaving the underlying cause untouched. As Dr. Tony Ebel explains it: “Don’t kill the messenger.” The imbalance that created the problem remains, which is exactly why ADHD medications wear off within hours and why stopping them produces a rebound effect that frequently leaves kids worse than before.

For parents, this means that addressing ADHD drug-free is not just possible, it is the only approach that gets to the actual root. The path begins with understanding how the nervous system became imbalanced in the first place: through what Dr. Tony Ebel calls The Perfect Storm.

Dr. Tony Ebel’s Personal ADHD Story: From Struggle to Superpower [00:00 – 09:26]

Dr. Tony Ebel: I most definitely have ADHD. This conversation is an easy, fast-flowing one for me personally because of my relatability and real-life experience, but also because I’ve spent the last 20 years not just learning to harness my ADHD into a superpower for myself, but studying the real science and root causes professionally. I’ve used that to help thousands of kids and adults overcome their ADHD-related challenges without dangerous, side effect-riddled drugs, medications that are Class II narcotic stimulants often given to kids as young as two or three years old.

ADHD frustration can be repackaged into excitement once you understand two things: first, that the conventional medical world’s view, that ADHD is a deficit and a disorder that must be medicated, is wrong. Second, that there is a real root cause you can actually address.

When you look at the history of this world, so many of the most impactful change-makers, past and present, have had ADHD. Wildly successful, happy people are taking their ADHD and making sure it is not a deficit, not a disorder, not a hindrance. They use it to create impact with an ADHD brain having a lot of fun all along the way.

That said, I will not skip over the struggles. The current overstressed, overscheduled, wildly toxic Perfect Storm world our kids are growing up in today is leading to more stressed-out, stuck-on-sympathetic fight-or-flight kiddos than ever before. There is a cliff that the ADHD brain and nervous system falls off of when stress, tension, and toxicity become too much, it shifts the nervous system into excessive fight-or-flight mode and the regulation side gets shut down.

“The way to get rid of the struggles of ADHD and turn them into a superpower is you have to keep digging all the way down to the foundational real root cause. And the foundational real root cause is not chemical imbalance.”

As a sophomore turning junior in college, my ADHD brain, which could always get assignments done in 13 minutes and ace a test without paying much attention, hit its struggle point when life got harder. I also wasn’t sleeping, wasn’t eating well, and went through a big family loss. My brain shifted from superpower to struggle for the first time. It turned out God’s path for me was studying genetics and molecular biology at exactly that moment, while falling in love with neurology and learning how the brain and nervous system work.

I decided to become a chiropractor, not for my ADHD, but for a bad baseball shoulder. Chiropractic fixed the shoulder, then fixed my brain. I went from a kid who couldn’t sleep to one who finally could. When I could sleep, I could concentrate in class. My grades went from low B’s and C’s to straight A’s and never went back. That is my story, and I share it because whether your child is 3 or 13, whether you are 39 or 42, that same shift is possible.

Why ADHD Is Not Just Genetics, And What “Environmental Factors” Really Means [09:26 – 18:15]

Dr. Tony Ebel: 95% or more of traditional medical providers still see ADHD as only a genetic disorder. They do not understand the real neuroscience behind it. They do not help parents deploy drug-free strategies that turn challenges into superpowers.

When people say ADHD is genetic, they mean: if two parents with ADHD have a child, that child is more likely to have ADHD. But this is not hardline genetic causation. There is no concrete proof that ADHD is “hardline genetic.” I was a genetics major in undergrad, I spent my college career studying this, and the answers pharmaceutical companies tell you are in the genetic hunt simply are not there.

Research has been clear for 20 years: ADHD is not solely genetic. There is genetic predisposition and slight wiring in that direction. But websites like WebMD, Healthline, and Mayo Clinic, after acknowledging “environmental influences,” never unpack what those actually are. The CDC and FDA only discuss socioeconomic factors, the stress that comes with lower-income communities. But stress is not relegated to certain communities or genetics. Stress is everywhere.

Here’s the real picture: two ADHD parents living full-speed raging bull, who have bought into the belief that their ADHD is a struggle and they need medication, they will tell their child the same thing. The genetic component is real. The family wiring skews in that direction. But the story of ADHD being a programmed deficit or disorder is not real, and it does not reflect the actual science.

The 10% of children now diagnosed with ADHD, and 3.3 million already on stimulant Class II medications, these stats are from before COVID. What COVID did to already-stressed kids and families doubled, tripled, and in many cases 10x’d their struggles. As of 2024, those numbers are worse.

“Your child’s brain isn’t poorly designed. That’s not how God made us. God made us different. There’s definitely genetic predisposition that skews toward ADHD wiring. But it’s not programmed to be a deficit. It’s not programmed to be a disorder.”

The Perfect Storm: How Prenatal Stress, Birth Trauma, and Toxins Drive ADHD [18:15 – 32:13]

Dr. Tony Ebel: The vast majority of kids today are exposed to more stress, earlier, than any generation before them. The Perfect Storm begins before birth.

Maternal prenatal distress, high-stress, high-anxiety, high-cortisol pregnancies, is more common by a large margin than ever before. Meta-analyses and statistical research confirm: high-stress pregnancies and birth interventions are strongly correlated with ADHD, autism, anxiety, obesity, asthma, and autoimmune conditions in children. The stress gets into the developing nervous system and brain early, when it is first forming.

Birth trauma from C-sections, forceps, vacuum extraction, and induction is the second factor. Rates of birth interventions have skyrocketed in the last generation or two.

Toxins arrive next. When a baby is colicky, constipated, not latching, and getting chronic ear infections, they are put on antibiotics and steroid-based medications. The number-one side effect of steroid medications for asthma and allergies is hyperactivity, irritability, and ADHD-like behavior. Overuse of antibiotics early in life is known to damage the gut, create what’s called “leaky gut,” and drive gut-brain inflammation. A damaged gut cannot properly create, assimilate, and absorb serotonin and dopamine, the very chemicals cited by the chemical imbalance theory.

The fourth factor is altered motor milestones. The raging bull, hyperactive ADHD kid sometimes goes through milestones too fast, walking at 8–10 months, climbing everything, needing constant movement to try to calm an overstimulated nervous system. The inattentive type may be slower through milestones.

Critically, the CDC literally removed crawling from its milestones chart because so many kids are skipping it. Crawling is essential to organize, calm, and prepare the brain for focus, concentration, social-emotional regulation, and communication. Removing crawling from the milestone chart instead of addressing why kids are skipping it is the system ignoring the root cause.

“They do not grow out of these things. They grow into subluxation, sympathetic dominance, dysautonomia, and what’s called nervous system dysregulation.”

The Two Types of ADHD: Raging Bull vs. Drunken Bull [32:13 – 37:35]

Dr. Tony Ebel: The medical world has expanded ADHD into six DSM diagnostic types. The more types they create, the more diagnoses they can give, the more medications they can prescribe. It doesn’t clarify what’s causing ADHD. It doesn’t help parents find drug-free solutions. It just keeps more families locked into the system.

Here is how we look at it: there are really just two neurological types.

The raging bull is the fight-or-flight, impulsive, hyperactive, wound-up kiddo. They love sports, climb everything, need constant movement stimulation to try and calm a brain that is full of sympathetic subluxation and fight-or-flight tension. They hit the skids around second to third grade, when school gets more serious and the nervous system, already over-wound, cannot keep up.

The drunken bull is the inattentive type, slowed down too much, cognitive and emotional processing grinding its gears. Posture is more sluggish, motor coordination more disorganized, reading comprehension and focus are challenges. This is the old-school ADD. These kids technically don’t have the H, they have the inattentiveness, and it comes from a different expression of the same underlying nervous system imbalance.

Medically, both are diagnosed as ADHD. Neurologically, both trace back to the same root: the autonomic nervous system imbalance where sympathetic fight-or-flight dominates and the parasympathetic vagus nerve, the rest, regulate, digest, and concentrate side, is suppressed. The bigger that gap gets, the bigger the struggle gets.

The Chemical Imbalance Theory: Accurate but Incomplete [37:35 – 50:56]

Dr. Tony Ebel: Here’s the most important neuroscience I can teach you about ADHD.

Dopamine and serotonin are neurotransmitters. Their literal one and only job is to transmit messages and control communication for the brain and nervous system. They are chemicals, called neurotransmitters, that follow what the nervous system tells them to do. The nervous system is the boss.

When there is dysautonomia, sympathetic dominance, and vagus nerve dysfunction, the entire downstream chemistry follows. The endocrine system becomes a mess. The adrenals, the HPA axis, the amygdala, the hippocampus, the prefrontal cortex, all of these get involved. But all of those things are downstream from the actual root cause neurological imbalance.

Healthline itself states: “Evidence suggests an imbalance in brain chemicals does not cause any specific mental health condition.” But they stop there. The full statement should be: the imbalance of neurotransmitters is first caused by excessive sympathetic dominance and suppressed parasympathetic vagus nerve tone. That condition is called dysautonomia and nervous system dysregulation.

Years ago, I was at a Parent University event, a full-day conference at local high schools, where I’d been invited to present on drug-free ADHD solutions. Afterward, a psychiatrist from the other presenters stopped me in the hallway. She told me that for years, families had been coming to her saying their child got better after starting neurologically focused chiropractic and asking to taper off medications. She asked: “How are you helping when it’s a chemical imbalance and you’re not changing chemistry?”

I asked her: “What chemicals are involved with ADHD?” She said, “Dopamine and serotonin.” I said, “What are those categorized as?” She said, “Neurotransmitters.” I said: “I fix the first five letters. We work on the N-E-U-R-O component of that imbalance. When you get the neuro system back in balance, the neurotransmitters and the chemistry follow.”

“Medications kill the messenger, not the cause of the imbalance. The chemicals are following what the nervous system tells them to do. The nervous system is the boss.”

From that day forward, that psychiatrist referred multiple children per month to our clinic, often before even putting them on medication, sometimes collaborating to taper kids who were already medicated. That collaborative care model is what the best care teams look like.

ADHD Medications: Side Effects, the Rebound Effect, and What “Working” Really Means [50:56 – 56:29]

Dr. Tony Ebel: ADHD medications are Class II narcotic stimulants, regulated by the FDA in the same category as cocaine. Their short-term side effects include sleep suppression, appetite suppression, tics, irritability, moodiness, stomach problems, fast heart rate, and high blood pressure. It’s reverse physiology: you take an already overstimulated nervous system and treat it with stimulants, overstressing it to such a degree that the brain basically shuts down.

When families tell me the medications “work,” I always ask: “Stop taking them for 30 days. Do you think they’d still work?” The answer is always no, they wear off in 4–6 hours, 12 at most. They are not working. They are suppressing, tricking, and manipulating the brain and nervous system. Which is exactly why there is a massive rebound withdrawal effect when patients get off of them.

The long-term risk is worse: the imbalance that was already causing the disorder actually gets worse over time as the medications lead the brain to believe there is balance when there isn’t. This makes the underlying problem harder to address later.

If your child is already medicated and you’re beginning drug-free chiropractic care: it is possible, it works, and we have specific clinical protocols for it. It may be a slightly longer road with a few more bumps, I will always be honest with you. But the protocols exist, and the outcome is the same.

If your care team is not open to drug-free options, or is not willing to work toward reducing medication dependence as the nervous system heals, you need a different care team. Parents, you are in charge. The neurologist, the psychiatrist, the pediatrician, they are not in charge. You are the advocate. You are the boss.

The Real Root Cause: Subluxation, Sympathetic Dominance, and Vagus Nerve Dysfunction [56:29 – 1:13:39]

Dr. Tony Ebel: The actual root cause of ADHD is a neurological imbalance that starts with sympathetic dominance. When a child goes through The Perfect Storm, a high-stress pregnancy, physically injurious birth trauma to the brainstem, early toxic exposures, stress gets hardwired into the developing brain and nervous system. Brains that are most plastic and impressionable during prenatal development, labor and delivery, and the first 3–7 years of life get stuck in this fight-or-flight pattern.

The chiropractic term for this is subluxation, stress that is physically stuck on in the neurospinal system. This is why forceps, vacuum, C-section, and induction are such a big deal. When we physically injure the neck and brainstem, we trigger a sympathetic response, and we compromise the vagus nerve, a cranial brainstem nerve that exits through the jugular foramen and runs down through the neck into the thorax.

The vagus nerve is sensitive to physical birth stress, to emotional stress, and to toxicity and inflammation. The kiddo who can’t latch, is refluxy, colicky, constipated, getting ear infections and antibiotics and steroids, the Perfect Storm is already brewing at 3 months, 6 months, 12 months, 18 months. The pediatrician says, “They’ll grow out of it.” They do not grow out of it. They grow into subluxation, sympathetic dominance, dysautonomia, and nervous system dysregulation.

ADHD ends up in the brain, but it doesn’t begin in the brain. The brain problems with the prefrontal cortex, hippocampus, and amygdala that researchers like Dr. Huberman and Dr. Daniel Amen discuss are real components of ADHD. But they are downstream results, not the cause. You have to keep asking “what’s causing that?” all the way down to the foundational root.

Neurologically-Focused Chiropractic Care addresses that root directly. Adjustments release tension from the sympathetic fight-or-flight system, activate and stimulate the vagus nerve and parasympathetic system, and allow the cerebellum, amygdala, hippocampus, and prefrontal cortex to work the way they are supposed to. That is the mechanism. That is why ADHD goes from a struggle to a superpower.

There is also a clinical correlation, meaning children with ADHD are significantly more likely to also struggle with sleep, digestion, constipation, eczema, immune challenges, chronic colds, asthma, allergies, fine and gross motor delays, speech issues, sensory processing difficulties, and sometimes even seizures. These are not separate problems. They are all downstream effects of the same underlying nervous system dysregulation. When you address the root, these challenges improve together.

“I study this stuff like the annoying toddler who keeps asking ‘why.’ Why is the prefrontal cortex out of balance? Why would the amygdala be overreactive? You have to keep asking that question until you get to the final foundational real root cause, and the real root cause is not from the brain. It ends up in the brain.”

INSiGHT Scans are the technology we use to actually measure nervous system dysfunction. Using thermal, EMG, and HRV technology, we can find it, quantify it, and locate it, not just describe it. These scans look deeper than any exam in all of healthcare and give us a clear clinical picture of what is actually causing a child’s challenges, not just what symptoms they are showing.

Frequently Asked Questions

Is ADHD a real disorder or just a different type of brain wiring?

ADHD is real, but it does not have to be a disorder. According to Dr. Tony Ebel, DC, CACCP, ADHD represents a distinct neurological wiring pattern that walks a fine line between struggles and superpowers. When the nervous system is balanced, that wiring drives creativity, high energy, and exceptional capability. When the nervous system is overwhelmed by stress, toxins, or birth trauma, that same wiring tips into daily dysfunction. The goal is not to suppress it but to address the neurological imbalance that turns the wiring into a struggle.

What actually causes ADHD if it’s not just genetics?

Dr. Tony Ebel explains that ADHD traces to a four-part Perfect Storm: maternal prenatal distress during pregnancy, physical birth trauma from interventions like C-section, forceps, or vacuum extraction, early exposure to toxins and antibiotics, and altered motor milestones (including skipping crawling). Together, these factors push the child’s autonomic nervous system into chronic sympathetic dominance, too much fight-or-flight, not enough regulation, which is the actual neurological root of ADHD.

Why don’t ADHD medications fix the problem long-term?

ADHD medications are Class II stimulant narcotics that suppress symptoms for 4–6 hours without addressing the underlying neurological imbalance. Dr. Tony Ebel describes them as “killing the messenger”: they manipulate dopamine and serotonin (neurotransmitters) without correcting the dysautonomia and vagus nerve dysfunction that caused those chemicals to get out of balance in the first place. Over time, medications can make the underlying imbalance worse, creating a rebound effect when discontinued.

Can chiropractic care really help a child with ADHD?

Yes, specifically Neurologically-Focused Chiropractic Care. Dr. Tony Ebel explains that chiropractic adjustments work on the N-E-U-R-O component of the imbalance: they release tension from the sympathetic fight-or-flight system and activate the parasympathetic vagus nerve. When the autonomic nervous system comes back into balance, the brain structures involved in focus and regulation, the prefrontal cortex, hippocampus, and amygdala, can function as intended. This is the only approach in healthcare that directly addresses the root neurological imbalance drug-free.

My child is already on ADHD medication. Can they still benefit from neurological chiropractic care?

Yes, though Dr. Tony Ebel is honest that the road may be slightly longer with a few more bumps for children who are already medicated. PX Docs clinics have specific clinical protocols designed for medicated children. The goal is to get the nervous system back in balance first, then work collaboratively with the prescribing provider to reduce medication dependence over time as the child’s neurology improves.
